Fee-for-Service Payment (Self-Pay)

Self-paying for therapy offers certain benefits that billing insurance does not. Consider this option for greater flexibility in your therapeutic services:

Privacy. Insurance providers require the completion of a diagnostic assessment and treatment plan as justification for therapy. These documents become a part of your permanent health care record. Self-pay services allow your health information to remain private and for you to opt out of diagnosis altogether if you and your therapist determine this is appropriate.

Simplicity. Insurance providers have made it more difficult than ever to know what to expect as it relates to billing. With self-pay services, there are no surprises. The price listed is always what you’ll pay. Payments will be charged to the credit card on file.

Agency. When billing insurance, your insurance provider may have limitations that impact your ability to access treatment. Session limits, excluded diagnoses, and excluded services, such as couple’s therapy, may be a barrier to you getting the most appropriate treatment. Self-pay services allow you and your therapist to determine the length and structure of services based on your needs and goals.

FBTA is happy to provide clients with the necessary billing information to submit to their Health Insurance, Health Savings Account (HSA), or Flexible Spending Account (FSA) for reimbursement.

Insurance

We are happy to bill your insurance for therapy sessions, if you prefer, and will make all reasonable efforts to verify your insurance coverage and obtain required authorizations. The amount your insurance covers may vary, and your costs and/or the need for a doctor’s referral, will depend on your insurance plan. It is your responsibility to know and understand your benefit coverage, as a verification of benefits is not a guarantee of claim payment. All benefits are subject to the terms and conditions outlined in the member contract with your insurance company. Payment Schedule

FBTA requires a credit or debit card on file for all clients billing commercial insurance plans and self-paying for services. Credit card information is encrypted to maintain security.

All account balances must be paid in full each month. This includes co-pays, deductibles, co-insurance, and any other fees incurred the previous month. On the 5th of each month or the next business day, the credit card on file will be billed for the full statement balance and a receipt will be sent to the email on file. To view a detailed statement or make a payment prior to the 5th of each month, visit the Client Portal.

Payments may also be made over the phone with a credit or debit card. If you wish to make payments using cash or a check, you may do so at a FBTA office. Please note, a credit or debit card is still required to be on file even if payments are made using cash or a check.

Services may be placed on hold if payment is more than 30 days past due and may resume when the balance is resolved. If payment is more than 120 days past due, FBTA reserves the right to send these accounts to collections.

Additional Fee Information

Please note that the following services are not billable to insurance:

Professional Consultations and Preparation of Forms and Reports: Professional consultation constitutes any time spent communicating with your therapist over the phone, other than for appointment information. Preparation of forms and reports constitutes any time that your therapist spends reviewing, gathering, printing and/or sending documents to you and/or outside sources. These services will be charged at a prorated rate of $200 per hour.
